Citizens Urged to Help Develop Three-Year Plan

Carroll County Health Department is conducting a Community Health Assessment for Carroll County. The department is collecting surveys from county residents as part of the assessment.
 
The survey results will help decide which three health priorities to focus on for the next three years and the strategies we need to use. Our current priorities are Substance Misuse, Mental Health/Frequent Distress, and Healthy Weight. These priorities can change or stay the same. Please share the survey within your organization, on social media, or through other promotions.
